Panchamukhi Hanuman is the form of Hanuman which represents five faces of the lord. There's an interesting story behind this. According to the legend, Lord Hanuman adopted the 5-faced avatar in order to rescue Lord Rama from Ahiravana – the lord of hell.

In order to rescue Lord Rama, Hanuman must extinguish the five lamps at a time in order to kill Ahiravana. So this made Lord Hanuman have five faces: Hanuman, Hayagriva, Narasimha, Garuda and Varaha

They signify:

Hanuman: Purity and success.

Narasimha: Courage.

Garuda: Dispenses poison.

Varaha: Wealth.

Hayagriya: Knowledge.
