The 43-year-old, who had previously won just one title on the PGA Tour finished on 22 under, a new record for the Mystic Rock Golf Course.
Frank Lickliter II, Tim Petrovic and Australia's Stuart Appleby finished in a tie for second on 268.
Rocco Mediate, Sweden's Jesper Parnevik and third-round leader Cameron Beckman shared fifth place, a further three shots back.
Heavy rain caused by Hurricane Isabel washed out play on Friday, meaning the field had to play 36 holes on Sunday.
Lewis began the third round in joint-12th, three strokes behind Beckman and Robert Damron.
With a third-round 68, Lewis actually lost ground as several players shot 64s and 65s to move up the leaderboard.
As the final round began, Lewis was seven shots behind Beckman but an eagle and two birdies in his first five holes set his charge in motion.
He made another birdie at the 9th to reach the turn in 31 and then had four consecutive birdes from the 11th.
He dropped a shot at the 15th but birdied the 16th and 17th on his way to his first win since the John Deere Classic in 1999. Bureau Report
