New Delhi, July 29: The provisional findings of a statutory inquiry into the derailment of 7201 Golconda Express on July 2, 2003, in Andhra Pradesh, has revealed that the accident occurred due to "delayed application of brakes". The findings of the Commissioner of Railway Safety, South Central Circle, R P Agrawal who conducted the inquiry, have been forwarded to the Ministry of Railways, an official release said today.

The accident at Warangal station on Vijaywada-Warangal- Secuderabad section of Secunderabad division of South Central Railway, resulted in the death of 21 persons, grievous injuries to 16 and simple injuries to eight persons.
